A standard annual service is £100. A full strip-down service, where the burner and heat exchanger are cleaned properly, is £160. Both include a written report.
Once a year. Most manufacturers also require an annual service to keep the warranty valid, skipping a year usually voids the cover.
It's worth getting an honest assessment. A well-maintained fifteen-year-old boiler can still run safely, but efficiency is typically 70–80% versus 90%+ for a modern A-rated unit. The gas-bill saving often pays back a new boiler within seven to ten years.
